Not something affiliated solely to the mind (thoughts are important, yes) but also to the physical (action).
Simply coming to a realization will not accomplish too much.

Hallucinations were mentioned in another thread.
I've personally had them because I went insane and I am, to some extent, still not totally... there.
But is it possible it's just something happening to the brain in that state... that has nothing to do with the external but just the brain going a little wack-o-lantern? Oxygen? Weird brain activity that causes hallucinations?
